Urology Northwest, P.S., participates in most insurance plans including Medicare. We accept Medicare
assignment. It is always best to contact your insurance company or the human resource department at your employer
to obtain a “preferred provider” list to insure we are a “participating” provider on your insurance
plan prior to scheduling an appointment. Some insurance companies will require you to obtain a “referral”
from your primary care provider before seeking specialty services.
As a courtesy to you, we will verify insurance
eligibility and benefits on all new patients prior to their appointment, if/when complete insurance information is provided
at least 2 days prior to the appointment date. We will need the name and address of the insurance company, policy
holder name and date of birth, Identification and group number.
For established patients
who are having a surgical procedure at one of the hospitals or ambulatory surgery centers with which we are affiliated we
will verify benefits and eligibility and secure any required “prior authorization”.
Urology Northwest,
P.S. will bill your primary and secondary insurance carriers for you directly. We utilize an electronic billing system
that processes claims daily. If your insurance carrier requires additional information from us to process the claim,
it will be provided immediately.
Urology Northwest, P.S. expects prompt payment for services rendered.
If your insurance carrier does not pay within sixty (60) days from the date of billing, we will look to you for payment.
Co-payments, deductibles or any co-insurance amounts are due at time of service. We accept cash, private
checks with proper identification, VISA, MasterCard and American Express.
Patients who do not have
insurance coverage, have a Health Savings Account, have amounts not paid by insurance such as co-payments,
unmet deductibles and co-insurance are expected to pay at time of visit or within 30 days of the date the insurance payment
has been received.
Patients having in office procedures such as vasectomies should be prepared to pay at least
1/2 of the cost prior to the appointment and the balance on the day of the procedures if insurance does not cover the service
or if Urology Northwest, PS has been unable to verify benefits and eligibility.
Payment plans are available with
the following guidelines:
- $250.00 payment at time of service-Patients with no insurance, insurance can
not be verified or benefits levels are less than 80%
- Balances can be paid over three (3) months from
date of service for non-insured patients; Balances after insurance payable within 30 days of insurance payment.
- Future services can be added to the payment plan but all services must be paid within three months from the applicable
date of service.
- Vasectomy - Full amount of $1000.00 paid on scheduling date is preferred.
If the patient has insurance a deposit of $150.00 is required when the procedure is scheduled. The patient pay balance
is to be paid within 30 days from the date of insurance payment. If the patient does not have insurance or if
insurance does not cover sterilizations then a deposit of $250.00 is required on day of scheduling and the balance of $750.00
is due on day of the procedure. If a patient cancels with less than 48 business hours notice or fails to keep the vasectomy
appointment the $150.00/$250.00 deposit is forfeited. The deposit is collected whether or not the procedure is covered
by insurance. If special payment arrangements are required the Business Office can be reached by dialing 425-275-5527
or 5528. Special payment plans must be arranged prior to the date of the procedure. To schedule an appointment

- Naturopathic Services-some insurance plans written in
Washington State cover naturopathic or complementary care. Urology Northwest, PS will call and verify eligibility and
benefits. We will bill your insurance directly for you. Patients with insurance coverage for naturopathic care
are asked to make their co-payment at each visit. IF the patient does not have naturopathic benefits or if benefits
have been exhausted a deposit of $75.00 is due at each visit. Medicare does not cover
naturopathic or complementary services.
- Vasectomy Reversal-Full amount of $2200.00 is due at time
the surgery is scheduled. Urology Northwest, PS does not offer payment plans for this service.
- Self
Referral appointments for such services as ED require a credit card payment of $50.00 at the time the visit is scheduled.
The balance of the $250.00 new patient deposit will be collected at time of service. If insurance covers the service
a refund will be sent within 15 days of receipt of insurance payment.
